I doubt that this is the problem, as the client's IP would be nat'd to a
public address. (Sorry Jake) ;)

 I'd check to see if the client's public IP isn't on an RBL. The smtp log
should show you if it's being rejected, providing you know the address:
# qmlog -lc "public\.ip\.of\.client" smtp
