First thing I'd check would be a firewall. The qmail daemon is
attempting to send the message, but the SMTP session is not completing.
Next I would try and telnet to your mail server from an outside source -
different network if possible.
Try initiating a SMTP session from this remote machine using telnet. Any
clues?
What changes have you made to the system from stock? Any special
patches, programs involving email, etc.?
Does your ISP allow port 25?
