Sorry for little mistake, I think there is no use of
SMTPROUTES file,
As depends on your qmail installation you can go through advice given by

Or just visit http://qmailtoaster.org/ for your reference. As per your
requirement linux system which is you are going to install with qmail
toaster. In that you have to write fetchmail to send mail to your exchange
server. and exchange server will send mail to outside through this qmail
server, That setting is in your exchange that send mail to qmail server.
Qmail server will send mail to outside world either through
your online server if you have took shared server, Or if you have your own
static ip, You can create A and MX pointer for the same..
If you want to send mail to outside world from exchange the
there should be some setting that you have to define your smtp host, i.e
your linux qmail server.
Once your mail come to your linux qmail server it will qmail job
to send that mail to respective domain of outside world.
