You can run qtp-newmodel, which will do everything for you.

Or you can download the appropriate binary rpm, and:
# service qmail stop
# rpm -Uvh clamav-toaster-0.98-1.4.5.*.rpm
# service qmail start

I've used the first method, but not the 2nd.
