Also, can you enable 'SIMSCAN_DEBUG=3' in tcp.smtp or 'export SIMSCAN_DEBUG=3' your smtp/run file. It would give an indication of whether clamd is exiting cleanly.

The script is super simple, and doesn't do anything that should affect clamav, but since you asked here's the reply to your questions:

I adapted Peter Samuel's qmail-queue-wrapper script to prepend `X-AuthUser: $ENV{'TCPREMOTEINFO'}` to every message. Here's it is: https://gist.github.com/quinncomendant/6a0a3a9d7699db48c9ade6f4942b8a92

All it does is prints `X-AuthUser: …`, followed by the entirety of the message as-is.

Since the script replaces the default qmail-queue, it is executed for all mail that makes it past simscan, local→local, local→remote, remote→local, remote→remote.
