The script is super simple, and doesn't do anything that should
affect clamav, but since you asked here's the reply to your questions:
I adapted Peter Samuel's qmail-queue-wrapper script to prepend
`X-AuthUser: $ENV{'TCPREMOTEINFO'}` to every message. Here's it is:

All it does is prints `X-AuthUser: …`, followed by the entirety of
the message as-is.
Since the script replaces the default qmail-queue, it is executed for
all mail that makes it past simscan, local→local, local→remote,
remote→local, remote→remote.
