I/O errors

I've been seeing these errors from time-to-time and am not sure what to 
make of it -- other than perhaps the client software not communicating 
properly -- it's likely outlook or something of that ilk:

Jun 15 12:02:17 drama popper[74306]: I/O error flushing output to client 
mvkelly at ip-67-65-65.la.ourcompany.net [216.00.00.65]: Operation not 
permitted (1)

These entries will be repeated several times for each instance.
