It's unacceptable most times and really does need some form of throttle.

passwd -l works well.

I don't believe any package pays attention to it.

I believe it should be patched. There are a few packages which are
obnoxiously set by default and a clueless user repeatedly hitting "check
mail" with a large inbox can affect system load badly, even in server
mode.
