I'm assuming that you're on medicaid?
If so, please call Community Choice Options, they help people go from nursing homes to living independently 1-877-856-0877. This is for people on Medicaid and in Medicaid facilities in NJ, if that's not your case we can find out more info based on your situation. The woman I spoke with is Marlene Hester with the Division of Disability Services. 1-888-285-3036.
