This is what we vent users use.

to use it though. It's just more common with vent users because we
often have no diaphragm movement. A good option for someone like
yourself would be the assisted cough. It works awesome. It's basically
a Heimlich(sp) maneuver on someone that can't stand. In a reclined
position or in bed, your assistant pushes the heal of their hand into
your diaphragm as you cough. You have to time your cough and their push
simultaneously. I'll try to find some articles for you. As far as
clearing crap out of your lungs is concerned, remember gravity is your
enemy. If you've been sitting for awhile and start to feel congested,
try sitting in a reclined /tilted position for awhile and then start
coughing.
