I would ask for a regime of medicine to control any possible infection  
before going in for oral surgery and I would suggest a general and that you be  
lifted into the doctor's chair for the procedure.  It won't be easy but  
necessary.  Bring a friend or two that can help move you there and back.  
Control any possible pre-surgery infection and post surgical infection as those 
 
both can be more serious then the actual removal of those teeth.

Fun  news, not only do I need a molar tooth pulled, but the tooth right 
next to it  broke and was pulled last year. I had a root canal on this tooth 
like 30 years  ago and he screwed it up. One of the posts is in the wrong spot

OK,  now the bad news.

I need to get all 4 wisdom teeth pulled too. Top ones  not bad, they are 
out. Both bottom ones are impacted going sideways pushing on  the root of the 
molars. Plus an infection in there. Normally they put you out,  but because 
of me being a quad and want to stay in my chair, he wants me  awake. Drowsy, 
gassed, but awake.
So they want to do it all at one  time...
