I did this when I put the Fiat 20vt engine in the 2+2...

Mondeo V6 4-hole discs, which were easy to come by, and the matching
callipers, which took some finding. The callipers are easy but the
carrier was the tricky bit.

The mounting holes aligned, but need drilling out on the hub which
makes this a non-undoable modification. I had to remove half a
millimetre or so from the hubs so the brake carrier didn't bind on the
discs.

Be aware that there are two sizes of disc - both should fit with a 15"
wheel but they have different callipers/pads/carriers... I have the
smaller due to difficulties finding the carrier for the larger.

In my case I went up to a 185/55R15 which needed an offset t25 to give
steering without binding on the inner arches, but that makes it very
close to the outer arch (the normal offset on a 13" wheel is T38 and
for ford 15" wheels is T55 which moves them inboard three quarters of
an inch). If you're keeping the same rolling radius, you'll want a 45
or 40 profile which will be easier to manage, though you'll still need
to think about the offset.
