No, it's a single poll change over.
There is one input (pin 30) and 2 outputs, one (87a) is connected to pin
30 when there is no power to the relay coil and the other (87) is
connected to pin 30 when there is power to the coil.
The coil is 85 and 86.
I would call this a single pole change over.

The far right one.
Just make sure you get the same type A or type B pin arrangement as
your current one.
But, first i would make sure you are getting power to the 30 contact.

But watch out are there are 2 different standards for the pin layout.
