I would look at Unity. I think that the best bet in the long run for doing gaming in QC would be to add in an actual "Game Kit" so to speak, or some kind of system/other editor to design levels, and then playback in normal QC, with some kind of ability to control aspects of the game. In some ways the OpenEMU for QC project was a good display of being able to load a game, and then "do" certain QC things to it.
