Sorry this doesn't clarify the other stuff you asked (which are all good 
questions that unfortunately I don't know off the top of my head).

Does glGetTexParameteriv with GL_TEXTURE_MAX_LEVEL and GL_TEXTURE_BASE_LEVEL 
indicate anything useful regarding mipmap levels?  GL_GENERATE_MIPMAP might 
also be useful to query.
