Thanks Herzliche. Your data confirms the data that I've found since posting the original message. I found this bug:
https://support.ntp.org/bugs/show_bug.cgi?id=870 And then I went back and looked at my server logs... it turned out that it failed to find the DNS entry (vs just not reaching the host). So according to the bug, if it can't find the DNS entry then it won't ever try that host again. Ah well. Thanks!
