I used to get these on occasion on my Merit RADIUS server even across my
local LAN. I think it was my server not coping with the responses fast
enough. My Ascend box seemed particularly picky about timing.

I've ended up billing on stop records. Sure I lose some calls now and then
but I found it was much better than the constant complaints of duplicated
sessions on the customers' bills. Your mileage my vary; I'm just sharing
my experience. :)
