Well the star is out and there is a new system as of the last printing of the leaders 
medal now can have numbers on 
the LMA.  One number at a time.  As you complete an extra course you move up in 
numbers.  You can only place a 1, 2, or 3

The pyramid pins were worn on the LMA back when the 5th section was new.
You would get a special pin for completing it.  The pyramid was worn on
the class A medal and a star was won on the class B to denote completion
of this 5th section.  Last I knew, the stars still came with the B/SA
class B LMA, but not the standard LMA.  The star is the same as worn on
the class B GMA.
