If the printer is moving 20mm up and down so it is inconsistent, it sounds
like the gears are loose/worn out and it is never going to line up.  Buy a
new printer.

Does a page eject for the HFCA/CMS-1500
form, prior to start of printing, fix the alignment
problem for the 2nd and subsequent pages?

Depending upon where we buy our forms, 
sometimes ejecting the first page is required 
for alignment of subsequent pages. This 
technique is irrational but it works.
