As others have mentioned a tuck key is the likely culprit. Having said that
you might want to review your code and/or database structure to ensure that
duplicates are not allowed;  "Duplicate Row" would still be allowed but the
record would not be stored until the proper fields have been updated. Over
the years  I have moved away from rules checking and do my own checking via
code.

Just wondering if anyone has ever seen this.
A form has a button to use Predefined- Duplicate Row At two locations over
the past few months the user finds they have 
500+ duplicate rows. As though they clicked the button 500 times.
(They didn't.)

My only guess is a network error where a NIC re transmits a mouse click that
"wasn't received" although it actually was.
