I have posted this before, BUT, if you report that the offending machine has
NT40 SPn, before you throw up your hands in dispair:
Reapply the Service Pack.
Reapply the Service Pack.
Reapply the Service Pack.
Reapply the Service Pack.

The service pack is only as good as the last time it was installed WITHOUT
installing any subsequent software on the machine.  This policy has "fixed"
my machine many times.
