Somehow, you have corrupted form data. Try this and see if it will correct the 
problem - no
guarantees, but it's worth a try.

SET NULL -0-
OUTPUT form.dat
UNLOAD DATA FOR SYS_FORMS3 WHERE #1 = 'formname'
OUTPUT SCREEN
DROP FORM formname
RUN form.dat
