FWIW, my last bike that had indexed bar-end shifters was always in need of adjustment, but only after an annual overhaul. Turned out that I greased the threads on the DT adjuster barrel/cable stop (when I shouldn't have) and so whenever I turned my handlebars, the SIS cable housing would spin the barrel out of adjustment.
