I would also check your tool holder. It sounds like the part off tool might be flexing as it goes in. Square of a peice of stock--face it and cut the sides down some. If it's not square with each other, you have problems, but if not, it's probably more the tool. Make sure the tool is square with the work(can't always use the tip, some have an angle to help with cutting) too.
