Try using the Flix Live 8.0.1.8.  I've added more info to the ticket as well
concerning our issues which are very similar.  Once you downgrade to 8.0.1.8,
you'll see it work much better, but still have occasional InsufficientBW
problems.

Also, once downgraded you'll notice that the encoder has no problems
reaching Red5, as the recorded video on the server works perfectly.  The
problem seems to be the server's ability to rebroadcast the live stream to
the subscribers.

The short of it is that a bigger live video stream (under VP6 + red5) has
longer tracts of functioning video, up to a few seconds at a time (as
opposed to a few frames at a time for a low bandwidth stream.)
