Hi Neil,

A few things may cause this from happening, I will list them by
likelihood

1.  The news article page is connected to multiple locations, which
have different publication package at each location.  When you publish
the news article page, or publish all following page from the an
ancestor page above the news article page, the news article page is
published multiple times, each according to each publication package.
The image is still on the server, just get moved around according to
different publication packages.

2.  Assuming you have "clean up live server", which removes page/
assets from web server when no longer in use, enabled in your project
variant.  Well, there is something that keep track of the files and it
may get corrupted over time, like a year of two.  To fix it, simply
create a mirror copy of the existing publication targets, and use the
new targets.  NOTE:  Please be careful when assigning new targets
because MS may go out to the old target and "clean up".  Do a full
site publish after.

3.  What version of MS are you currently use?

Please also note that "always replace media files" option increases
your publication transfer time and "Clear page builder cache before
publication" drastically increases your publication time and often
your page rendering time in SmartEdit and page preview after publish.
