By "online help", do you mean web-based help, or platform-native integrated help? If the latter, for what platform? What platform are you developing on?
There are no open source help authoring GUIs that I'm aware of (unless you want to use a wiki). If you want an open-source solution that outputs to web-based help with a table-of-contents and built-in search, consider DocBook XML. The web-help output was recently added to the DocBook Tool Chain. I have a few posts on my blog about free or cheap help-authoring tools.
