    The Scala doc needs to include details on how column indices and names are 
organized, and how duplicates are treated.  How about, for simplicity, we do 
not allow any duplicates, even between indices and names?  Something like this:
    ```
    Indices and names are not allowed to have any repeated features.
    ```
