If you have, say, "cover.jpg" in a folder of MP3s, does iTunes use it
as the album art?
Does it have to be "cover.jpg"? Oh, and how do you make Max output the
filename "cover.jpg"? I've tried loads of times but it only seems happy
with making the album art filename out of preset fields, such as "{Album
Artist}-{Album Title}.jpg" etc.
