Personally, I've had much better results out of USB MIDI dongles than with 
internal cards.  Out of three cards I've used, all three have had their MIDI 
driver broken by routine maintenance updates of the kernel.  My USB dongle, 
however, has never failed to work perfectly.

Mine was discontinued a long time ago, so I can't make specific 
recommendations as to what new options are available out there.  In general, 
though, the USB audio driver, which includes USB MIDI support, seems to be 
more solid than average.
