So we finally found a solution to this. It looks like one of the records in the sessions table got corrupted, which was causing all session writes to fail, and so logins would never "take". Flushing and repairing that table resolved the issue.
How it got corrupted in the first place, no one knows. I suspect it was bribed by an unscrupulous lobbyist.
