Maybe ctrl-alt-bkspce needs to be enabled and is diabled by default
now. You can also enable ctrl-num-/ or so to actively break grabs.

It sounds like an active grab.
To debug that, you could enable AllowClosedownGrabs and/or
AllowDeactivateGrabs (see xorg.conf(5)) and try those keycombos when it
happens, to see if they break the grab.
Could be a bug in X (given it seems to happen with new xorgs only) or in
urxvt (grabs are tricky, but I would expect some emssage) or in both.
