Remember those Informed Consent Certifications that had to be filled out
for every human being tested in a Science Fair Project, even if was a
survey? Well, the guideline changes instituted in 2000 make an exception
for paper and pencil surveys. If you are not surveying anything that may
cause stress in the subject, skip the forms, however, endorsements are
always needed. If you do have to get Consents, there is no need to send
them in to either region or city officials prior to displaying at either
fair. Consent forms just need to accompany the project while it is displayed.
