I've been using self-hosted Trac for myself and on small (<5ppl) and medium-sized (~15ppl) teams and we like it.
I like Trac for the simplicity, no frills and integration with just about anything you can think of. I tried Privotal Tracker and could not get into it, too many steps. Assembla just feels overblown for something that should be straight forward. Plus what Matt said. I've also tried using Lighthouse, I like it but I get the same features from Trac, granted without the nice GUI, but Trac is free. Redmine is somewhat like Trac but written in Ruby instead of Python. I've spoken with a few people who use it and they love it. I think it really depends on your budget and your requirements/ constraints. Maybe you can provide some more details so that people can give you a better answer.
