[ozmidwifery] hep b @ birth

Hi ,

A number of staff in our unit have commented that babies who have Hep B
immunisation just after birth seem much more unsettled for the first
24-48 hrs than those babies who don't have the immunisation .Has anyone
else noticed this or are we just imaging things ?? Our unit has only
recently changed to offering Hep B immunisation after birth at the same
time as the Konakion,we used to give it on day 3 or 4. Interested in any
comments or if anyone knows if any studies have been done .

Hi Raelene,
We have a policy that allows co-sleeping.We had more concerns about
babies falling out of bed(did actually happen) rather than them being
smothered by mums so our policy just makes sure the bed rail is up  on
which ever side of mum the baby is sleeping with a pillow against the
bed rail so bub doesnt slip through.We regularly have bubs in bed with
mums .Works well for both mums and bubs.

Hi everyone,
I need some help! I'm trying to formulate a policy regarding
co-sleeping and want to offer alternative sleeping arrangements for
mothers and babies whilst in hospital. Does anyone know of a special
cot that has been developed that allows the baby to sleep with mum but
in a separate cot that is attached to the main bed. I've seen pictures
of babies using a biliblanket in a cot attached to the bed in this way,
but can't find any information. Can you help.

Hi Kristin ,

SSD is silver sulphadiazine or Silvazine is the trade name.It has been used as 
the treatment for burns for more than 20 years it has good antibiotic 
properties and encourages moist wound healing .Have not seen any side effects 
from it in 30 years of nursing.

Hi,

There is an excellent article on www.health-e-learning.com  written by
Robyn Noble which deals with infant lactose intolerance and colic .I
have had excellent results using her dietry changes etc .Also she
usually reccomends the use of small quantities of bowel flora
replacements such as Nourishing flora

[ozmidwifery] Re :Politics

Hi,
I work in a NSW public hospital as a midwife and our local state member
has given us consent  forms to get the mothers to fill in to give him
permission to to send them a congratulatory card after the birth of
there baby.He is a member of the current government party and holds his
seat by a very slim margin .Am I being cynical in thinking this is
politics sinking to another low to get votes ?Or is he just being nice
and geting to know his new constituents???
Is it happenning at any other hospitals?
 I really dont think it is my job to do this for him or any other
politician .What do others think??

 Fully agree with you I have no problems taking babies to the nursery at night 
for mums that are totally exausted and giving them that little break they need 
.Wrap the babies up, put some Mozart on the CD player and they will often sleep 
for hours .Then change them  and take them out for a feed when they wake or 
mums can come to the nursery and have a cuppa and chat with other mums that are 
up doing the same thing .Mums are encouraged to feed in the nursery at night 
were we have nice comfortable recliner chairs and it doesnt disturb the other 
mums in there room .There is a lot of communication goes on between the multis 
and primips when they get together to feed .We also have a video player in the 
nursery were they can watch videos like Follow me mum  etc. Its also 
interesting to notice how much better the mothers are in themselves and with 
their bubs after those few hours of sleep.

I work some night duty in a small unit  if mothers ask me to 'mind' their 
babies  take them back for feeds overnight then I do, willingly.

I'm heavily into nurturing women, odd eh ??

Hi ,
We have not done rectal temps on babies or infants for more than 18
years. About that time we got terumo axilla thermometers and have
continued to use them They are very accurate .I did some comparisions
when we first got them between the axilla thermometers and rectal temps
and found they were almost identical so have never done a rectal temp
since .The Terumo thermometers calculate core temp and you wil find they
actually go up and then drop back to the babies core temp

there is a systematic review on the topic of rectal vs infrared ear
temps in children on cochrane database
the conclusion is as follows:
'The authors concluded that although the mean differences between
rectal temperature measurements and ear temperature measurements were
small, the wide CIs mean that ear temperature is not a good
approximation of rectal temperature. They suggest that when body
temperature needs to be measured with precision, infrared ear
thermometry should not be used in preference to rectal measurement,
which is the established method'
 
personally i think its a very invasive and (for older children)
embarassing thing for them to go through and as it says above should be
done when the temp needs to be known with precision ie when they are
very ill, not just as a routine measure. im not sure why they didnt
include axilla temps..

Interesting, Carina and Denise,

I feel breastfeeding advocates have been hoisted on their own pettard
- for so long we sold the message that breast is best.  Not the obvious,
that breast is normal; formula feeding is not, and some risks.  Risks
which we may choose to accept, or may have to accept.  Unfortunately
guilt has become a blunt instrument with which to bludgeon
breastfeeding advocates, and prevent sensible discussion of the real
issues.

As Denise said, occasionally, like any other part of the body
occasionally breasts don't work.
My son and I have asthma. (I guess I gave it to him as part of our
genetic heritage) Our lungs don't work properly. Some-times I supplement
him with oxygen, and everyday we both take drugs.  I don't feel guilty
about that.
With my first child (not the asthmatic one - he of course was fully
breastfed til 6 mnths) I got into a complete pickle.  Work, lack of
sleep, difficulty adjusting to my new situation.  I was easily conned
into believing my breasts didn't work.  The formula flowed freely. 
Unfortunately the breastmilk did not seem to do likewise.  I scraped
through - breastfeeding and artificial feeding. 
Subsequent children have taught me that my breasts worked fabulously -
but I did not have a good idea of what normal was.(they taught me
that, too)  I recieved liberal lashings of poor information, until I
managed to access the support of the Australian Breastfeeding
Association
I don't feel guilty about formula feeding.  I did the best I could
under the circumstances, which were trying to say the least.  
In my volunteer work as a breastfeeding counsellor and my professional
work as an infant feeding consultant I work with women every day.  Not
all manage to breastfeed, despite all our efforts.  Sometimes I don't
know why.  Some-times I do.  We don't live in a culture where
breastfeeding is normal and understood.  However, I do believe women
deserve the information that formula has risks, and encouragement and
information to breastfeed.  I dispense info on weaning, and work to
promote breastfeeding as normal and to increase society's knowledge of
breastfeeding.

I help mothers as much as I can, understand why it didn't work for
them, grieve for a time of life lost, reflect on their experience and
enjoy their baby, and not feel guilty about the decision they made, or
had to make.

that I'm not breastfeeding regardless of how many people told me it was
best.

  My opinion is that some health professionals don't know how to
support women to breastfeed adequately and in covering their own
feelings of guilt about this they 'pretend' that it's the mother they
are trying not to make feel guilty.

  To give you lots of different opinions on guilt and breastfeeding do

runs a breastfeeding clinic)
TakeFish oil 4000mg
Evening primrose oil 2000mg
Vitamin C  4000mg
Take these every day while breastfeeding.
Take half in the morning and half at night with meals.If there is any sign of breast 
redness or pain increase the vit C to 8000mg.Some forms of vit c are easier tolerated 
than others and the mother may need to experiment with this.I have found 
www.goldenglow,com.au very reasonable in there prices for these products and there 
delivery has always been quick. Hope this helps   Lyle

After reading about Blackmores SSPC on this list awhile ago I have suggested its use 
to a number of women  with severe hyperemisis with very good results.Vomiting seems to 
settle completely and they say they feel great.SSPC is only available with a naturpath 
prescription or from a chemist that has a naturpath(which we are lucky to have in this 
town).Have asked Blackmores about its safety in pregnancy and they say its totally

severe morning sickness  is now 9+ weeks pregant? SShe has tried 
acupuncture  chinese herbs as well as being admitted for IV  rehydration  
being given Maxalon, Vit B6  even dexamethasone. I thought I'd consult the 
list for any more ideas.

Some Obs I know use Dostinex to prevent lactation in mothers who don't want to 
breastfeed.It needs to be given on day1 or 2 post deliver .A dose of !mg is given stat 
and it totally stops any lactation .Dont Know about its safety or long term effects 
and its very expensive , about $75 for the one dose .Has any one else had any 
experience with this product and its use Lyle
PS I would prefer to see the body left to naturally work through its cycle as 
suggested by others on this list

need some protien and some fat with all three meals Lyle

Is this in a hospital?  Check the mothers diets!  What is on the menu for
them?  I am sure that most hospital diets are not breastfeeding friendly.

Hi,
Has anyone read Dr Tom Brewer's website at www.blueribbonbabies.org ?
He was an obstetrition in the USA who found that by putting mothers on a good balanced 
diet he eliminated all PIH and abruptio placenta and  had a greatly reduced  incidence 
of prem labor and  miscarriages . Does any one else have any information or experience

Hi,
we use fenugreek often in our unit with good results.The studies indicate a minimum of 
3 grams a day is reqired for any effect ,ie to maintain supply. To increase supply up 
to 6 grams is required , these doses are usually divided up and given tds with 
meals,ie 2grams with breakfast lunch and dinner .If it causes diarrhoea,which is about 
the only side effect,cut the dose down.Fenugreek causes perspiration to smell like 
maple syrup and the mother needs to take enough fenugreek for this to happen for it to 
be effective  Lyle

Could you please help me by giving some information about the use of fenugreek in the 
period after birth. A client has asked about its use, but I don't know much about it. 
Does it increase or decrease milk production? Does it assist uterine involution? Does 
it assist iron uptake? Should it be used in conjunction with other herbs? What is the 
best way to use/take it?  As I said I don't know anything about it.

Dear listers
Does anyone else out there do a routine 10 min Apgar score on babes? I am having a 
long running argument with our Quality Improvement Coordinator that this score is not 
routine and most cases meaningless. It just means more paperwork with little impact on 
the way we practice. She insists that it is an Aust wide clinical indicator. Anyone 
else got any ideas ?

My sister in law had a bub that was breech ,she tried most of the suggested exercises 
with no success.I had attended one of Andrea's seminars just before this and had heard 
of moxibustion and suggested this to her .I got details from Andrea and my sister in 
law tried at 37 weeks with great results baby turned and stayed tuned and was born as 
cephelelic presentation at term

The website www.childbirth.org has a lot of information on biirthing plans that seems 
a little more current .It gives samples ,info on how to write one ,opinions of mothers 
who used them and a lot of other information.Although they are written with an 
American view they still have a lot of useful  ideas that we can use

yes, I think we all need to be aware that the birth plans on the internet all 
seem to be about 20 yrs out of date. We don't use eye drops anymore in 
America at least not in Washington or California. And does anyone give 
perineal shaves anymore? But those items are still on the internet birth 
plans. I guess they just need to be updated. We seem to need a template for 
everything.
