Siri constantly interrupts me and says sorry but can't hear me; I'm not

If it does, then if you are listening to an audio book, something is triggering
Hey Siri, which could be anything that sounds remotely like Hey Siri.

I often have the iPhone next to me on the table when I wake it with a tap on
the screen and then tilt it a bit towards my face. It mostly unlocks.
Holding the phone shortly away from your face after a failed scan and then beck
again sometimes works, but not always.
