Connection refused indicates that the recipient's e-mail system might be doing reverse DNS lookups. Do you have a correct reverse DNS record for the IP address you're using? I could try that, but that doesn't tell what went wrong when Exchange sent the message. There are a number of ways I can send it (HELO VS EHLO just for example). Exchange will tell you in the tracking logs, if you have message tracking enabled. Connecting from the Exchange server as James suggested is generally faster than looking in logs.
