 May be it will not help but. I had the same problem with my Radeon VE card.
 It worked fine and then just stoped working with the same message. The
 problem was in the TV out. If your card has a TV out and you have connected
 something to it, just unplug the cable and then reboot your machine twice
 and try to  start the X server.

basicly, it will not relay on the monitor to supply the refresh rates 
information.
this is happening when using a plug and play monitor type in XF86Config-4, 
and not specifying a monitor type. XFree cannot actually tell which monitor 
you are using, and will relay on getting the refresh rate information from 
your Device section - your graphic card.

This is from nvidia's README:


Option IgnoreEDID boolean
Disable probing of EDID (Extended Display Identification
Data) from your monitor.  Requested modes are compared
against values gotten from your monitor EDIDs (if any)
during mode validation.  Some monitors are known to lie
about their own capabilities.  Ignoring the values that
the monitor gives may help get a certain mode validated.
On the other hand, this may be dangerous if you don't
know what you are doing.  Default: Use EDIDs.

the problem also happend with older drivers, and then I've decided to upgrade 
to the latest.
this is obviously a missconfiguration problem, which i just cannot put my 
finger on.

You might want to add such a line:
 BusID  PCI:1:0:0
into your 'Section Device' in the XF86Config file.
it helped for me.
also you might want to upgrade the version of the nvidia driver.

and changed the driver name from nv to nvidia (had to do that for playing

the only way to load X now, is to change it back to nv and to disable glx 
support.
NVidia drivers are installed (from the nvidia website) and the NVdriver module
