Try pressing VO with Q to turn first letter quick nav on and off. This
is different from just pressing your left and right arrows together.

I see nothing regarding hotkeys in the VoiceOver Rotor. I have everything else 
selected in the rotor, shut down and restarted VoiceOver and still no hotkeys.
