I would recommend, if you are already using DocuSign for official
signatures (and possibly document management), you should find a form
builder that has a DocuSign integration. There are a few listed on the
DocuSign website.
Here is one to start with

And, hinted at this above, but what are you using for document management
and electronic agreements? I would be sure to find a form builder that
works with that system so you are able to automate some workflows instead
of doing different work in 2 or three systems and trying to keep track of
everything.

fields and signatures, dates, etc. Very easy UI.

If you're looking to add to the list of technologies you can use to accomplish 
this type of task you should also have a look at:

https://www.foxitsoftware.com/pdf-editor/

This company focuses on PDF automation and workflows.  I'm not sure you will 
find it any simpler to build the documents / forms using this software but this 
company provides a complete workflow process that may save you time on the 
daily tasks after you put in the effort to create the forms.
