Hello Hope,

To rename a document you have to select Documents, then in the list of 
documents, place one finger on the one you wish to rename, toggle VoiceOver 
off, double-tap with that one finger, then toggle VoiceOver back on again. 
You'll be in an edit field which is the file name. Type the new name, then tap 
Done. If you've already given the document a name, you'll need to delete the 
original name or the new one will be appended to it.

For this reason, I do not consider Pages truly accessible. I got a refund after 
I discovered this problem.
I've reported this bug to apple and I really wish that others would as well. 
This is not the way it should work. You should never need to turn VoiceOver off 
while using an app. Again, I would consider this a bug.
If you look at the different note apps, you don't need to deactivate VO, 
perform a function, then reactivate it.
This is an Apple manufactured app and it shouldn't work like this. They didn't 
follow there own guidelines that they have published for apps to work properly 
with VoiceOver.
