I've noticed a similar problem with an iPhone 6s and an iPad air 2.

I don’t’ know if it’s maybe something to do with iOS 9.1 as I hadn’t come 
across this problem in earlier versions of iOS. Either way, it’ is incredibly 
annoying and disruptive to my work. This morning for example, both phones are 
connected fine with Vo coming through as it should. I fully expect this not to 
be the case later in the day though. 

 Oh well.

This happens occasionally with my wired earphones, too.  I can’t quite figure 
out what causes it.  Sometimes it’s after a phone call and sometimes not.  The 
only fix is to turn off my phone.  Unplugging and replugging the headset 
doesn’t fix the problem.
