Yes, Ni-cad batteries are effected on either end. What people are less aware 
of, however, is that Ni-cad batteries also suffer if you discharge them too 
much. Flatlining a Ni-cad battery is as bad as over-charging it.

Yes, in fact, it has come out that the Braille Note actually has ni-cad 
batteries sealed inside it, so yes, with said products, it is essential to 
follow ni-cad charging requirements. Rather amusing too, considering how much 
the product costs and how much the replacement of batteries costs.

Ok, this is a li-polymer battery. the way to charge and discharge it is quite 
different from ni-cad batteries. With ni-cad batteries, any time you do not 
fully charge the battery, a chemical change occurs within the battery that 
causes it to lose a little bit of its total capacity. This is not the case with 
the li-ion and li-polymer batteries.

When the iPhone battery reaches the high 90% range, the system switches to 
trickle charge. It then takes significantly longer for the battery to charge to 
maximum capacity. At 100% consider the battery to be full. Trickle charge 
continues until the battery is completely full. Then, if left on the charger, 
the battery loses charge until it is just at the switching point between 99% 
and 100%, at which point, the trickle charging circuit activates again. Apple 
seems to have adjusted the 100% mark down a little to cover the start and stop 
trickle-charge cycle.

I'm not sure what kind of battery is used in a Braille Note, but LI 
batteries, which are now the most commonly used batteries in mobile 
devices, won't suffer if they're not completely charged. I know NiCad 
batteries suffered from the memory effect, so they liked to be all used 
up before they were charged up again, but I'm not sure even they were 
negatively impacted by not being completely charged up.

Usually when I wake up in the morning, and my iPhone has been charging 
overnight, it says there is 98% battery. I have no idea where that 2% goes.

Like Chuck, I don't worry about it.

Why are you worried about taking the phone off of AC before it's fully 
charged? this won't hurt a LI battery. If it's not fully charged, it's 
close, close enough that I wouldn't think it would effect use time.
