I'm running clamav with mailscanner and I'm seeing lots of problems with clamd
crashing.
I did use some of the additional sigs from sanesecurity but I've had to remove
these as when using them, mail just doesn't get processed. The servers are
quite heavily used.

If that is of no help, we need more info in order to debug. I usually chase
clamd bugs by setting "Forground yes" in clamd.conf and use gdb. Also "Debug
yes" in clamd.conf may provide additional clues.

Which "make" are you using? Does Clam-AV require GNU Make as well? You
might be able to install that as "gmake". (GNU Make and BSD Make aren't
entirely compatible - I'm not sure where AIX Make lies on that spectrum.)

timeouts. This issue developed just this week.
I found that setting
OfficialDatabaseOnly true
helped considerebly--clamd loads in a few seconds. I'd really like to
get back to using unofficial databases, but not right now.
Last time I tried it with an empty list, and it still took 5 minutes for
clamd to start.

Your email headers say that you're using Outlook. Either Outhouse (or perhaps
even a malignly-configured MTA) strips them or simply doesn't show them. Your
firewall has nothing to do with it
