Yes, add the IP to the smtpaccess database with the value AUTH_REQUIRED=1.

Yes, it works perfectly. I configure tb to use port 465 (ESMTP SSL)
I do the same thing, but use TLS over esmtpd-msa (587). And I did this
for exactly the same reason as the original poster... so that only
authenicated (and secure) connections can relay. :-)

I apologize for polluting the list with what turned out to be a simple
mistake on my part. The actual problem was that esmtpd.authpam was
missing a 'd' in the filename!
