I thought I should at least try testing without any extensions enabled.
That fixed it on xenial :-)
The offending extension was QuoteAndComposeManager 0.3.32.3 (1/16/13).
Disabling this resolved the issue. My other extensions (especially
Lightning, which is important to me) are OK.
I have tried version 45 on a different system, running trusty 14.04, and
it works properly.
What can do I do help resolve the problem on my 16.04 system?

That is the official GNOME Logo, not our old logo.
Likewise the GNOME docs come from upstream, and are not Ubuntu GNOME
specific. While we could patch it to use the Ubuntu GNOME logo, I am not
sure that is really necessary. Any decision to do so would have to come
from the artwork team.

went to add a second admin user via the control center immediately after

On laptops, screen brightness and
each DM has its own unique method for setting autologin.
I dont know if the gnome-control-center UI ever worked? If it did maybe
we dropped some Ubuntu patch. Does the UI work on Unity/Lightdm combo?
