I tried to reproduce this in LibreOffice 4.2.4, and it appears to be
fixed now.
1) open Original.doc supplied in the bug report
2) save as saved.doc
3) close document
4) reopen saved.doc (still in LO)
5) second page header is present (unlike new.doc supplied in the bug report)
So let's mark this

Jaak, thank you for taking the time to report this bug and helping to
make Ubuntu better. However, I am closing it because the bug has been
fixed in Trusty Tahr.
This is a significant bug in Ubuntu. If you need a fix for the bug in
previous versions of Ubuntu, please perform as much as possible
for me not reproducible with LO 4.0.2.2 (Win7 Home, 64bit)
Is this a Linux issue only or was this already resolved in the meantime?
Steps Done:
1. Open the attached file Original.doc with MSO 2007 and LO and compare it
Result: You can find the header in LO as in MSO 2007. But contrary to MSO
Sorry, I have to amend my last comment.
If I open the attached file Original.doc with LO and save it again as
a doc file and afterwards reopen it with MSO 2007 and LO, then in both
programmes the heading is missing. In this case I can confirm a missing
header with LO 4.0.2.2 (Win7 Home, 64bit).
