The solution I came up with is to -- as you suggested -- output this "endofdoc"
reference as the last block in TOC, but to make the font color="white". By doing this,
the previous TOC item was printed in full and the last TOC dummy item, though incomplete, is output
as invisible text.

Glad I could "help" :-)
I suspect that, in the long run, this isn't really a "solution" anyway,
for those who've got as their last TOC item something that doesn't
happen to be on the last page. I guess this is one of those strange
bugs... :-p

Jon,

Can you merely use for the
last item (and put at the last item)?

Open a bug in bugzilla, and append a small, self contained
FO document which demonstrares the problem.

There is currently no way to make it perfect, but you can improve
it a bit with a table

The leader form the second cell will probably overlap the other leader. You
might need to adjust the cell width a bit so that the dots match exactly.

I have the same problem (last page number citation in a TOC does not
appear), and also this one: the last page number citation on *each page* of
a (multi-page) TOC does not appear.

Also, (which of course depends on your type of document),
try to use a page-sequence per chapter or section.
I also had that last id in my ToC mysteriously disappearing,
until I set up a page-sequence per chapter.
Since the page-number-citation refers to the title in the
chapter, it lies first in the flow and this seemed to help,
at least for me.

I'm not using multi-column layout or keeps, or lists, and the block IS an
immediate child of the flow.
