Re cross-linking documents at the heading level in PDFs, try the AutoBookmark
plug-in available for Acrobat.

I encountered what sounds like the same problem. The key for me is
never to change the selected printer from Adobe PDF. If I need to
print, I save as PDF and print the PDF.
