I think you're going to have to have them add a comment balloon next to the
xref.

In the first case (catalog), I didn't set up the files so have no idea why
that approach was taken. While the chapter start master page was built with
a disconnected chapter-head frame, only a few of the actual book (body)
chapter documents were that way--that is, all the others' chapter-h

Of course I revise any annotated cross-ref in the source FM document.
But the cross-references must be reviewed and annotated, if
necessary, in Acrobat. I give these PDFs to my clients for review and
THEY must be able to annotate them.

It there a fast way to produce a list of all fonts used in a document and
reference what page they are used on? Or 'Search and Replace' - when there was
no tag was used in formatting.

could you explain how your files are set up - and in particular this erroneous
file - why do you use separate disconnected frames for your headings?
Do you place the frame you mention on the Body page or on the Master Page? Do
you use multiple flows in your documents?
