Another method I use quite a bit is to just hook a drive up to a Linux system
or pop a bootable Live CD in the machine and boot into a Linux environment to
use the DD command. It can be as simple as this:

point, I found the inode in a directory which usually is mounted for
an (ez-) jail via nullfs.
This proves that problems with jails, nullfs, and fs corruption are possible.
So, they can not be excluded up front in your case too because nullfs is just
a simple path translation.

What you should do right now is to get some recent general or security cd/dvd
with chkrootkit and rkhunter and run them from that external read-only media.

This is incorrect - /mnt is defined under all standards (Filesystem Hierarchy
Standard, Unix directory structure) as contains filesystem mount points.
So, lost+found dir should exist under root dir as /lost+found.

Just finished installing BSD 5.0. Had an old CD, made an ISO image,
attached it as a CDRom to a new VMWare machine, booted form it, and can log
on.
It's been a few years, since I've done the install or used UNIX. I'm a bit
